• * ml per bottle: How many millilitres are in the bottle(s)?
• * State of Manuf.: What state does the wine come from?
• * Qty: How many bottles of wine are there?
• * Unit Value: What is the value of each individual bottle of wine?
• * Total Value: What is the total value of the number of bottles?
To work this out, calculate the Qty x Unit Value
• HS Tariff No.: This is to be completed if the article is being sent
for commercial reasons
• FDA Product Code: This is only to be completed if the article is
being sent for commercial reasons
• * Total Value & Currency: This is the total of each individual
subtotal total value and the currency it is declared in.
• * Reason for Export: state the reason why you are sending this article
eg. Gift, Commercial reasons
• Please state if the goods are Duty Drawback: This is only to be
completed if you are sending the wine for commercial reasons
• Please state if goods are hazardous: Please note that you cannot
