If you are sending wine by Express Courier International or Express Post International to the destinations below, the Wine Export Declaration & FDA Notification Form must be included in the item. Failure to include may result in delay.

SENDER:

• * Company/Contact Name: The sender’s company name (or) sender’s name
• * Address: The sender’s street address
• * City/ State: The city and state that the article is being sent from
• * Postcode/ Country: The postcode and country (AUSTRALIA) that the article is being sent from
• * Phone/ Fax: The sender’s contact telephone number (or) fax number

RECEIVER:

• * Company/ Contact Name: The recipient’s company (or) contact name
• * Address: The recipient’s street address
• * City/ State: The city that the article is going to. If you have the state – please provide
• * Postcode/ Country: The country that the article is going to. If you have the postcode – please provide
• * Phone/ Fax: The contact telephone number (or) fax number of the recipient

SHIPMENT INFORMATION:

• * Consignment No: The Australia Post consignment number
• No. of Cartons: This will always be 1
• * Weight of Article: The weight of the article that you are sending
• Destination Import Permit No: This is to be completed if the article is being sent for commercial reasons
• Order No: This is to be completed if the article is being sent for commercial reasons

FULL DESCRIPTION OF WINE(S):

• * Label Description: The name of the bottle(s)
• * Red/ White: Is the bottle(s) Red or White wine?
• * Still/ Sparklg: Is the bottle(s) still or sparkling wine?
• * % Alc/ Vol: What percentage of alcohol is in the bottle(s)?
• * ml per bottle: How many millilitres are in the bottle(s)?
• * State of Manuf.: What state does the wine come from?
• * Qty: How many bottles of wine are there?
• * Unit Value: What is the value of each individual bottle of wine?
• * Total Value: What is the total value of the number of bottles? To work this out, calculate the Qty x Unit Value
• HS Tariff No.: This is to be completed if the article is being sent for commercial reasons
• FDA Product Code: This is only to be completed if the article is being sent for commercial reasons
• * Total Value & Currency: This is the total of each individual subtotal total value and the currency it is declared in.

• * Reason for Export: state the reason why you are sending this article eg. Gift, Commercial reasons
• Please state if the goods are Duty Drawback: This is only to be completed if you are sending the wine for commercial reasons
• Please state if goods are hazardous: Please note that you cannot send Hazardous goods with Australia Post – please contact 1800 007 678

• * Company/ Contact Name: The company name (or) contact name
• * Name (in print): The sender’s name in full
• Position: The position in the company of the person who is sending this article
• * Signature: Please sign here
• * Date: The date that this article is being lodged with Australia Post
